A Case Report on Effects of Therapeutic Exercise after Posterior Lumbar Interbody Fusion(PLIF)

Abstract

INTRODUCTION
The purpose of this study was to present the effects of therapeutic exercise after posterior lumbar interbody fusion surgery(PLIF).
METHOD
This case was 37-year-male patient who had operated PLIF due to herniated lumbar disc. The subject performed the balance test, lumbar extension strength test, and subjective pain scale(VAS) test 3 times; pre-surgery, 8 weeks after the surgery, and 16 weeks after the surgery. Rehabilitation exercise was started from three days after the surgery, and had keep going to 16 weeks after the surgery. Also, rehabilitation exercise program were applied with brace to 8weeks after the surgery, and were applied for 8-16weeks after the surgery without brace.
RESULT
The Balance of pre-surgery was 36.8% deficit between left and right, 29.4% deficit between anterior and posterior. However, The Balance deficit after rehabilitation exercise for 16 weeks was almost within normal limit. The lumbar extension strength was increased at each angle. Especially, Isometric strength at lumbar 0 degree extension was increased about 127% from 58lbs to 132lbs and at 60 degree of lumbar extension was the least increase about 48%. Also VAS was decreased from 7cm to 2cm. This patient didn't felt any pain during activity and daily living after rehabilitation exercise program.
CONCLUSION
The rehabilitation exercise program for 16 weeks after PLIF increased balance, lumbar strength and decreased subjective pain score. We concluded that rehabilitation exercise after PLIF operation must be increased as periods and controled by symptoms.
